This form is for McCoy College Business Graduate Students requesting one of the two options:

Course Substitution Request: This is for students who would like to take a course that is not listed in their degree audit/plan. The course can be in McCoy College or a non-business course.
Course Transfer Credit From Another Institution: This option is for students requesting transfer credits from another university. Before filling out this form read through the Transfer Credit section of the Course Credit page on the Graduate College website to verify that you meet all necessary criteria for transfer credit.

Note: this form also requires the syllabus for the course requested. Please reach out to the course academic department or instructor for a copy of the syllabus.
